It also takes time, effort, and skilled circulating staff. Releasing the traction and repositioning the hip into flexion by use of a standard traction table requires the continuous attention of an unscrubbed assistant and can result in significant ankle dorsiflexion if care is not taken when flexing the hip. This relaxes the anterior capsule, providing maneuverability of the arthroscopic instruments, and improves visualization of the anterior and anteroinferior femoral neck. This, in turn, should result in a more accurate resection. The increased freedom of movement and ease of placement of the limb in space allow excellent access for fluoroscopy and, importantly, improved visualization of the femoral neck while the surgeon is assessing cam lesions, as well as efficient dynamic assessment of areas of impingement. Once the position of the leg is satisfactory, releasing the pedal locks the leg in place. The surgeon can flex, internally or externally rotate, adduct, or abduct the leg into any position desired, limited only by the patient's anatomy and the position of the post (for adduction) ( Fig 2, Fig 3, Video 1).

Stepping on the pedal to release the nitrogen allows free movement of the leg. With this system, placing the leg in an ideal position for peripheral-compartment dynamic assessment is much easier and can be completely controlled by the surgeon and his scrubbed assistant. When the central compartment work is complete, the traction is wound off by turning the crank counterclockwise. The operative site undergoes standard sterile preparation and draping. The contralateral foot is placed in a well-padded boot and positioned in 40° of abduction and slight flexion and external rotation to allow space for the fluoroscopy unit to obtain true lateral images of the hip. The operative limb is placed in 0° to 5° of hip flexion and abduction relative to the pelvis with 15° of hip internal rotation. The nitrogen-powered articulated positioner is attached to the traction bed with a special adaptor, compatible with the type of traction arm used. The foot is placed in a traction ski-type boot ( Figs 1A and 1B) attached to a Spider articulated hip traction system (Spider Limb Positioner Smith & Nephew, Andover, MA). The patient is moved down the operating table, and a well-padded radiolucent perineal post is placed between the legs and 5 to 10 cm off-center toward the operative limb.

Hip arthroscopy is performed with the patient in the supine position on a traction table (XRT3000 manual operating table, OTA/4102 carbon fiber saddle, OE/1059 traction frame Denyers International, Victoria, Australia), and bony prominences of the foot and ankle are well padded.
